As a Registered Nurse Case Manager, you will play a crucial role in providing exceptional care and support to our patients in the comfort of their own homes. Plus, we provide tech support to automate your paperwork and save you time!Responsibilities:- Conduct comprehensive assessments of patients' health status and individualized needs to develop personalized care plans.- Coordinate and manage patient care in collaboration with physicians, therapists, and other healthcare professionals.- Provide skilled nursing care, including medication administration, wound care, and monitoring of vital signs.- Monitor patients' progress and update care plans accordingly, ensuring interventions align with their evolving needs.- Educate patients and their families on disease management, self-care techniques, and available community resources.- Advocate for patients' rights and ensure their preferences and goals are integrated into the care plan.- Maintain accurate and up-to-date documentation of patient assessments, care provided, and outcomes achieved.- Collaborate with interdisciplinary teams to facilitate seamless transitions of care and improve overall patient well-being.- Adhere to regulatory guidelines, company policies, and industry best practices.Qualifications:- Valid Registered Nurse (RN) license in Oregon.- Minimum two years of nursing experience, preferably in home health or a related field.- Strong clinical assessment and critical thinking skills.- Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals.- Ability to prioritize and manage multiple cases efficiently.- Demonstrated compassion, empathy, and sensitivity in providing patient-centered care.- Knowledge of Medicare guidelines and home health regulations is desirable.- Proficiency in electronic health record (EHR) systems and basic computer skills.Important:- Schedule: On call.- License/Certification: RN (Preferred).- Willingness to travel: 75% (Preferred).- Work Location: On the road.- Job Types: Full-time, Part-time.Job benefits:--- Flexible schedule.--- Mileage reimbursement.Your potential leader(s):Kyle Nielson, Associate Founder.
