Phys Therapist - RESPONSIBILITIES (listed are included but not limited to) - Plan, review, and implementation of treatment programs based upon initial and interim evaluations.
- Communication and collaboration with the patients, family members, caregivers, interdisciplinary professionals, and other internal/external elements both orally and in writing.
- Adapt assessment tools and treatments to the complexity of the diagnosis or disabilities and demonstrates the clinical reasoning necessary to identify the need for further in-depth specific assessment of function and utilization of non-standard methods and techniques.
- Documents plan of care and care given in patient's medical record.
- Monitors patient's response to interventions, modifies treatment plan, and re-evaluates as indicated to meet short-term and long-term goals.
- Expected to interpret findings from examination and evaluation, select appropriate test and measures, and integrate findings in the physical therapy plan of care for the full range of patient populations.
- Fits devices for patient needs and instruct patients/care providers in the use of these devices.
- Develops home programs for patients as needed prior to discharge and instructs patients, family, or caregivers in implementation of home program.
- Provide consultation, as needed, to peers and other staff responsible for the care of the assigned patient.
- Provide clinical oversite and collaboration with physical therapy assistants.
- Provide patient care which is appropriate to cognitive, emotional, and chronological maturation needs of young adult, middle adult, and older adult patients.
