Job Summary: This position is a licensed registered nurse role designed to provide direct nursing care to patients in a post-acute care setting. RN will work closely with other permanent and temporary staff and assist in their duties as needed. Essential Job Duties: - Essential job duties include, but are not limited to assessment, patient and family communication, administration of medications, planning and delegation of nursing interventions, assistance with ADL's, admission, discharge, wound care, monitoring chronic and acute conditions, medication administration, appropriate documentation; accurate, prompt and professional communication with healthcare team including nursing staff, providers and ancillary team members; completion of any and all training modules,utilizing chain of command as needed, and working within scheduling parameters.
- Serves as a role model and resource for LPNs, CNAs, Med Aides and Caregivers following the mission, vision, and values of the organization and fulfills other job duties as requested within the Scope of Practice.
Functions: - Using evidence-based practice, clinical decision-making, compassion, and therapeutic communication, the registered nurse leads efforts to create the safest patient environment and the best patient experience.
- The nurse delivers and coordinates patient-centered care.
- The registered nurse is responsible for providing and coordinating comprehensive patient care through the appropriate utilization of assessment, diagnosis, planning, implementation, and evaluation, in accordance with established standards, policies, and procedures.
- Makes it a practice to take charge of self-development by soliciting feedback, setting priorities, and then acquiring or strengthening skills and knowledge through coursework or other methods.
- Makes it a point to put new skills and knowledge to use on the job in a timely and effective manner.
- Assists with orientation and training of others as directed.
- Demonstrates behaviors commensurate with the Vision, Mission, Values, and Behavioral Standards of SnapCare.
Communication/Knowledge: - Effective written and verbal communication skills; able to communicate with various individuals both inside and outside the organization.
- Possesses strong interpersonal skills including the ability to maintain professionalism in stressful situations, openness to learning, ability to identify and utilize resources in an unfamiliar environment, and strong conflict resolution skills.
- Proficient with computer systems.
Education/Experience: - Qualified candidates will have graduated with an Associate Degree of Nursing (ADN) or higher from an accredited nursing program.
- At least 6 mo of LTC and/or post-acute care inpatient care nursing experience is required.
- Skills: Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency meets the licensure requirements in your home state; or for Non-National License Compact states current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works.
- Primary Source Verification Required.
- Current Basic Life Support for Healthcare Provider status according to the American Heart Association.
