DescriptionEssential Job FunctionsPerform clinical responsibilities in a manner that provide effective, age-specific patient care while achieving optimal outcomes.Complete evaluations within appropriate time frame per policy.Set goals with measurable outcomes and appropriate time frames.Demonstrate appropriate use of various treatment modalities in patient care.Recommend discontinuation of treatment when appropriate.Initiate discussion with physician and other medical team members when appropriate to modify the treatment program.Supervise COTA and Home Health Aide in a professional manner.Maintain records that are accurate, concise, timely and legible in accordance with agency and system policies.Document therapy care in format in accordance with practice setting including: Oasis assessments, evaluations, treatment provided, response to treatment, patient progress, education, family conferences, discharge planning and discharge summary as appropriate.Complete reassessment at appropriate intervals and documents accordingly.Communicate post-discharge needs to appropriate resources.Document patient care that is consistent with guidelines and requirements of agencies and third party payers.Educate all patients and families in a manner consistent with their level of understanding and therapeutic needs.Involve patient and family members in establishing a plan of care.Provide patient and family members with educational materials as needed.Document education and training as well as patient and/or family level of achievement with training provided.Capture maximum revenue for quality service provided while maintain established standards of care.Maintain adherence to established productivity standards.Complete and document patient information or/visits on a daily basis.Provide relevant information to insurance provider to insure proper authorization is secured prior to initiation of treatment when indicated.Assure treatment does not extend beyond last covered date for physician's orders or insurance authorization.Complete recertification process when neededUtilize supplies in a fiscally responsible manner.Maintain responsibility for accurate reporting of weekly work schedule.Promote continual performance improvement.Demonstrate individual initiative for performance improvement by attending meetings and actively discussing ideas for advancement.Collaborate with patients, families, DME providers and other health team members to provide excellence in patient care.Communicate regularly with other team members via verbal communication and established written and/or electronic methods.Participate in interdisciplinary team meetings as indicated.Serves as a liaison for patients with DME providers.Implement a self-directed plan for professional development.Staff schedules require flexibility as they are dependent on patient care needs, which may include evening and weekend hours, as necessary to meet demand in their primary area or the needs across the system.Travel to all Excela Health Home Care & Hospice service territories as patient census dictates, in order to provide quality necessary care where patient need is greatest.Cognitive ability to function and concentrate on the essential and specialty functions of the job at a high level.Achieves thoroughness and accuracy when accomplishing an essential or specialty function of the job.Ability to think and concentrate for an entire shift.Specialty Job FunctionsUnderstand the Hospice philosophy and perform care accordingly to enhance the quality of life of the terminally ill patient.Maintain and demonstrate an understanding of the Hospice benefit and comply with regulatory conditions of participation.Function as a member of the Interdisciplinary group to develop a plan of care that addresses the physical, psychosocial, emotional and spiritual needs of the terminally ill patient and their family.Follow established guidelines for pain and symptom management to enhance quality of life.Serve as a liaison for the patient with DME providers.Participate in supervision of students as appropriate.Participate actively in professional activities.Participate in orientation of new Homecare personnel.Other related duties as assigned.Required QualificationsBachelors degree in Occupational Therapy.Strong leadership ability, good organizational skills, independent thinking and decision-making ability, analytical problem solving skills, sound judgment, excellent oral and written communication skills, must be able to function in a team environment, and knowledge of legal aspects and liability of nursing practice.Preferred QualificationsKnowledge of computer applications, research experience, knowledge and/or certification in specialty treatment program (Low Vision, Lymphedema, Pediatrics, Hand Therapy, Women's Therapy, Occupational Medicine).Minimum 1 year experience or 10 months of experience with two months of on-the-job training in the past 2 years in a Hospital, Skilled Nursing Facility, Home Health Agency or other relevant experience.Masters degree in Occupational Therapy.License, Certification & ClearancesCurrent licensure to practice in the State of Pennsylvania required.Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program
