Whether you're considering your next career move or your first, you should consider Lehigh Valley Health Network.$10,000 Sign-On Bonus; 24-Month Commitment Summary Provides mental health assessment, diagnosis, treatment, and crisis intervention services for adult and/or child members who present with a broad range of mental health needs. Coordinates care for patients and assists them in accessing services and resources that will help maintain them in the community in the least restrictive environment. Job Duties - Oversees all utilization management activity to include authorization of services as well as management of denial and appeal process.
- Ensures all patient discharge planning documentation is clear, concise, timely, and congruent with treatment team recommendations.
- Conducts clinical assessments for interpretation and treatment intervention with patient and families as directed by treatment plan.
- Provides case management and care coordination services for clients/patients.
- Intercedes on behalf of the individual patients to assist them in achieving and maintaining community stability. Acts as a liaison between patients, referral sources, and funders.
- Manages all elements of mental health - voluntary and involuntary commitment processes as regulated by Pennsylvania Mental Health Procedures Act. (chapter 5100 regs)
- Participates in quality improvement activities for the department and or program.
