About This Licensed Practical Nurse (LPN) Position HopeHealth is seeking an experienced Licensed Practical Nurse (LPN) for its Rhode Island team. HopeHealth is a fast-growth and innovative not-for profit healthcare organization, focused on caring for people with serious illness. This is a full-time, 40 hour per week position, with 1:00pm-9:30pm hours, 5 days a week. There is a rotating weekend commitment, located at the Hulitar Center in Providence This position is eligible for a bonus of up to $2,500. What will you do as a Licensed Practical Nurse (LPN)? - Conducts assessments, assists with admissions and discharge planning for IPU patients for home and nursing home and nursing home patients. Assesses patient and family status based on the physiological, psychosocial, spiritual, and emotional needs presented.
- Implements the plan of care under the direction of the nurse case manager and attending physician.
- Demonstrates skill in interpersonal relationships with Hospice patient, families, co-workers and community support services.
- Maintains accurate records in designated manner to record observations, problems and nursing actions which reflect the care given to Hospice patients What are the qualifications for a Licensed Practical Nurse (LPN)?
- Current RI Nursing License
- Two years hospice or oncology preferred
- Must have well developed nursing assessment and clinical skills including solid documentation and medication dosage calculations
- LPN required
- Hospice/Palliative care nursing certification preferred What are the job benefits?
