FULL TIME RN CASE MANAGER - Johns Island South Carolina

As a Full Time RN Case Manager, here's the big-picture view of what you'll do:

  • Maintain a specified case management load.
  • Complete initial/comprehensive patient/family assessments to determine hospice needs.
  • Participate in on-call duties as defined by the office on-call policy.
  • Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician.
  • Make necessary revisions to the IDG plan of care as the patient's/family's status and needs change.
  • Coordinate care among IDG members and other healthcare providers external to the IDG.
  • Evaluate and coordinate the need for services provided to the patient/family in accordance with the IDG plan of care.
  • Direct the services provided by and supervise the LPN/LVN, Hospice Aide, and Homemaker assigned to the patient/family.
  • Facilitate the consideration of another level of care (i.e. inpatient, respite, continuous care) when indicated by the patient's/family's status/needs. A few must-haves for the Full Time RN Case Manager:
    • Graduate of an accredited school of nursing.
    • Licensed to practice as an RN in the state of South Carolina. License is active and in good standing.
    • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
    • One (1) year of hospice care experience.
    • One (1) year of precepting or training experience preferred.
    • CPR certification.
    • Able to provide proof of valid driver's license and auto liability insurance. Benefits for the Full Time RN Case Manager:
