Responsibilities

• Telephonic monitoring and documentation of medical treatment to ensure it meets established guidelines.
  
• Monitoring patients' progress toward recovery to identify continuing care needs and facilitate discharge for specified populations.
  
• Collaboration with Medical Management Physician leadership and internal departments to ensure seamless care coordination.

  • Education
    
    
    
    • Required: Registered Nurse (ADN or master's degree in Social Work)
      
    • Preferred: Certification in area of specialization, BSN
      
      
      
    • Experience
      
      
      
      • Required: Five years nursing experience or three years' experience in medical social work with an emphasis on case management of high-risk patient populations.
        
      • Preferred: Proven program development skills
        
        
        
      • License(s)
        
        
        
        • Required: TX RN license or TX LCSW license, valid Texas Driver's license
          
        • Preferred: Case Management Certification
          
          
          
        • Special Skills
          
          
          
          • Required: Proven communication and problem-solving skills; Computer literate
            
          • Preferred: At least 2 years in area of specialization, 2 years of Case Management/Utilization Review experience
