Key Responsibilities

Conduct assessments and evaluate patient data to develop and implement nursing care plans
Engage patients using Motivational Interviewing techniques to promote adherence to care plans
Document patient interactions accurately and participate in quality improvement efforts


Required Qualifications, Training, and Education

Nursing degree from an accredited college or university
Minimum of 5 years of broad clinical experience
At least 3 years of experience in home care, care coordination, or telehealth
Strong computer skills and ability to navigate electronic health records
