• Shift Details: EITHER 24hr Sat/Sun/Mon OR 32hr Fri/Sat/Sun/Mon Hartford HealthCare at Home is seeking a Weekend Visiting Nurse (LPN) to join our team. As a LPN, you will facilitate communication between the patient and the interdisciplinary team, provide education and promote self-advocacy and independence, and utilize state-of-the-art technology to document skilled services provided. The ideal candidate will have a valid CT LPN license, at least one year of LPN experience, homecare experience is preferred, a valid driver's license, reliable transportation, and computer proficiency.
