Venture Forthe is expanding into the Endwell and Syracuse region, and we are looking for a dynamic Assistant Director of Patient Services to join our team. POSITION SUMMARY:The Assistant RN Director is responsible for direction, coordination and direct supervision of the agency's Field RNs and Field LPNs. This position provides in-field professional supervision and on the job training to RNs and LPNs. The Assistant RN Director is responsible for tracking and ensuring that the RNs assigned to them complete work in a timely manner and visits occur within policy. This role will provide training and support for new hires in the form of orientation as well as on-going training tailored to the individual employee's specific needs. The Assistant RN Director will oversee all LHCSA obligations in regards to their team. The Assistant Director will have regional oversight of the RNs and the participants serviced and will provide support for all other RN Supervisors when their ADP is unavailable. All policies and procedures for LHCSA operations will fall under the Assistant Director to enforce and uphold. This position will communicate policy changes and job expectations to his/her staff.The Assistant RN Director is responsible to travel to all offices that they oversee to meet with and observe the nurses that they oversee on a regular basis. This is a full time position with some overtime. ***Sign on Bonus up to $10,000***This number is based on the number of hours the individual is working on a part-time basis. For example, if working in a Full-time 40 hours/week capacity, the individual would be awarded $10,000. If the individual works Part-time 20 hours/week, the amount that would be awarded would be $5,000. Requirements: Key Responsibilities: - Serve as the foundation of our nursing team in the Syracuse region.
- Assist in building a caseload, which will eventually be delegated to other nursing staff.
- Conduct client home visits and supervise Personal Care Aide (PCA) staff.
- Oversee and support the PCA Training Program Education.
- Travel between Endwell and Syracuse regions to ensure quality patient care and team coordination.
- Be the face of Venture Forthe in the Syracuse region, establishing our presence and building lasting relationships.JOB DUTIES
- Supervises RNs on their team and provides support[OK1], guidance and direction through regular contact with their team members and regular review of job performance
- Ensures team members perform within LHCSA, Agency and departmental policies, meets deadlines[OK2]and provides quality assurance [OK3][OK4]
- Ensures Team Field RNs Perform patient re-assessments in the home. Reassesses patient needs for services on a continual basis and discusses any changes in the plan of treatment with patient physician/family.
- Performs semi-annual on-site competency visits /evaluations with nursing personnel as part of supervision activities.
- Provide employee development for the nurses that are under their supervision.
- Interprets and reinforces agency policy and procedures to professional and paraprofessional staff.
- Acts as a resource for the on-call RNs on a rotating basis by providing direction and oversight for nurses on-call.
- Reviews reports from all involved disciplines, re-evaluating needs and supervises all involved disciplines in the plan of care.
- Participants in departmental meetings and prepares reports as requested to evaluate the job performances and productivity of the departmental employees.
- Participate in a regular review of departmental policies and updates as necessary.
- Reviews aide competency reviews.
- Communicates with referrers/discharge planners to review eligibility for agency services and to develop and coordinate the plan of care.
- Provides assessment of newly referred patients, assesses health needs and eligibility for home care. Identifies the appropriate level of care required by the patient.
- Ensures Medical Orders and written reports are provided to the patient's physician, as necessary.
- Reviews written documentation and summaries of the patient's status and progress as needed for continuation of home health services. Reviews Team's clinical documentation monthly
- Plans, facilitates, and documents patient's discharge from agency.
- Becomes familiar with all resources necessary to provide a complete care program for the patient, e.g., respiratory, DME, etc.
- Maintains updated professional knowledge and participates in patient education programs to ensure optimum quality of patient care. Participates in the agency's in-service program.
- Participates in the agency's CQI Program, annual policy/procedure review, as requested. Performs Quality Improvement duties, completes audits, and evaluates results with DPS recommendations for quality improvement.
- Reviews and responds to complaints, grievances and incidents.
