Key Responsibilities

Serve as a liaison between patients, caregivers, and healthcare providers, managing care coordination and appointment scheduling
Collect and document patient information and clinical data while adhering to standardized procedures
Collaborate with the multidisciplinary Care Navigation Team to enhance patient experience and support


Required Qualifications

High school diploma or equivalent required
Preferred experience in handling medical records and patient communication
Familiarity with Electronic Health Record (EHR) systems and patient scheduling platforms
Self-motivated with the ability to prioritize and complete tasks efficiently
