Key Responsibilities

Evaluate member needs through post-discharge assessments and medication reconciliation
Develop care/service plans and collaborate with healthcare teams to support member transitions
Facilitate education and resources for members to promote wellness and improve care quality

Required Qualifications

Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
