- Acute Rehabilitation Case Manager
- Virginia Mason Franciscan Health Rehabilitation Hospital is a state-of-the-art, 60-bed inpatient acute rehabilitation hospital dedicated to the treatment and recovery of individuals who have experienced the debilitating effects of a severe injury or illness.

- Role (RN, Tech, LPN, PT, etc.): Case Manger (SW, RN, PT, OT, ST, RT with CM experience)
- Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist with appropriate CM experience.

Job Requirements - CM experience
- CM certification of any type preferred
- Certification in Case Management or Rehabilitation Nursing preferred
- Commission for Case Manager Certification (CCM)
- Association of Rehabilitation Nurses (ARN) certification
- American Case Management Association (ACM)
- Board Certification in CM by the ANCC e.g.: RN-BC
- Effective oral and written communication skills in English, additional languages preferred
- Basic computer skills in Excel, Word, Outlook, etc. required
- Good organizational skills, time management skills and analytical ability

Unit Specific Information - Common Diagnosis/Treatment: Acute rehab diagnoses, most common is Stroke but multiple medical debilitation; post surgical; with an ability to participate in acute rehab criteria (IRF level of care)
- Personality Fit: Communicative; Able to work well with diverse team of professionals; Understands how to balance patient care advocacy with stewardship of resources; professional communication in at times challenging discussions with families and patients around difficult insurance/health care realities.
