Key Responsibilities

Conduct 15-30 minute telephonic/chat coaching interactions focused on behavioral change
Engage with members diagnosed with Type 2 diabetes to support health improvement
Collaborate with a Care Team to personalize programs and review member data for glucose management


Required Qualifications

National Board-Certified Health and Wellness Coach (NBC-HWC) or other Coaching Certification
1+ years of coaching/behavior change experience with proven results
Bilingual in Spanish
Experience working virtually with groups and individuals
